Please see below to locate the form requested from you.  The forms are year specific so make sure you choose the correct form for the correct year, 2011-2012(for current academic year) or 2012-2013 (for academic year beginning August 2012).  DO NOT submit any forms not requested.

Documents are requested as required by Federal Student Aid accountability processes. All documents received will be scanned and linked to the student's file. Documents are retained in the office for one year and then archived according to document-keeping requirements.

IMPORTANT: Please follow the guidelines below when you submit requested information. 

1. Follow directions and make sure all documents are complete and legible. Incomplete forms will not be processed and will delay your aid.
2. Please submit only copies and keep originals for your records (we cannot make copies for you). Only exception: U.S. naturalization papers*, for which we must verify the original.
3. Do not send documents by email, as email is not a secure means of communication.
4. Please do not resend additional copies unless specifically asked to do so.  Sending duplicates will delay processing your file.
5. Choose a method for submitting requested documents:

12-13 Board of Governors Fee Waiver Application
Waives an eligible student's enrollment fees (tuition). DO NOT file separate BOGFW application if you have submitted a FAFSA. 

12-13 Dependency Status Appeal
For applicants who would like to request review of their Dependent status for consideration as Independent for financial aid need analysis.

12-13 Dependency Override RENEWAL
For students who were previously granted a Dependency Override by Butte College. Students with initial requests must use the Dependency Status Appeal (see above) and provide documentation as requested on the form

12-13 Dependent Verification Worksheet
To be completed by dependent financial aid recipients selected for federal verification. Requires both parent and student information and signatures.

12-13 Eligible Non-Citizen Verification 
Form to enable students who are eligible non-citizens to document their non-citizen eligibility for Federal Student Aid programs.

12-13 FAFSA Dependency Status Form
Form to enable students, who answered “yes” on the FAFSA to being orphaned, in foster care, a ward of the court, an emancipated minor, in legal guardianship, or homeless, to document their Independent status for financial aid purposes

12-13 Parent Appeal for Unusual Circumstances (available July 2)
For parents of applicants who would like to request adjustments to their student’s federal student aid application based on unusual circumstances. Through an appeal process, we will determine if the circumstances will result in a change to student’s aid eligibility.

Academic Probation Contract
For students who have been notified that their SAP Appeal for Extenuating Circumstances has been approved for Probation. In order to receive financial aid after appeal approval, students must return this completed form to the Financial Aid Office as soon as possible.

Application for Summer 2012 Pell Grant
For students enrolling in Summer Session at Butte College if: (1) they have a 2011-2012 FAFSA on file with our office; (2) they are eligible for Pell Grant and have remaining 2011-2012 eligibility (i.e., did not use full-time Pell in fall and spring); and (3) their program requires completion of courses in Summer Session.

Course Evaluation
For students that are required to report academic standings in current classes.

11-12 Dependency Status Appeal

Excess Units Petition
For financial aid recipients who have failed to meet the SAP time frame requirement. Students disqualified for exceeding time frame requirements may appeal by submitting an Excess Units Petition. Deadline for submitting petition: October 30 for Fall; March 30 for Spring.

11-12 Homeless Status Verification
Form to enable students, who answered “yes” on the FAFSA to being homeless, to document their Independent status for financial aid purposes.

For financial aid recipients who have failed to meet SAP criteria after a WARNING period due to serious illness or injury, death of an immediate family member, or other catastrophic or compelling situation beyond their reasonable control.
