Please see below to locate the form requested from you. The forms are year specific so make sure you choose the correct form for the correct year, 2011-2012(for current academic year) or 2012-2013 (for academic year beginning August 2012). DO NOT submit any forms not requested.
Documents are requested as required by Federal Student Aid accountability processes. All documents received will be scanned and linked to the student's file. Documents are retained in the office for one year and then archived according to document-keeping requirements.

12-13 Board of Governors Fee Waiver Application
Waives an eligible student's enrollment fees (tuition). DO NOT file separate BOGFW application if you have submitted a FAFSA.
12-13 Dependency Status Appeal
For applicants who would like to request review of their Dependent status for consideration as Independent for financial aid need analysis.
12-13 Dependency Override RENEWAL
For students who were previously granted a Dependency Override by Butte College. Students with initial requests must use the Dependency Status Appeal (see above) and provide documentation as requested on the form

To be completed by dependent financial aid recipients selected for federal verification. Requires both parent and student information and signatures.
12-13 Eligible Non-Citizen Verification
Form to enable students who are eligible non-citizens to document their non-citizen eligibility for Federal Student Aid programs.

2011-2012 (Fall 2011 - Spring 2012 - Summer 2012)
Academic Probation Contract
For students who have been notified that their SAP Appeal for Extenuating Circumstances has been approved for Probation. In order to receive financial aid after appeal approval, students must return this completed form to the Financial Aid Office as soon as possible.
Application for Summer 2012 Pell Grant
For students enrolling in Summer Session at Butte College if: (1) they have a 2011-2012 FAFSA on file with our office; (2) they are eligible for Pell Grant and have remaining 2011-2012 eligibility (i.e., did not use full-time Pell in fall and spring); and (3) their program requires completion of courses in Summer Session.
Course Evaluation
For students that are required to report academic standings in current classes.

Excess Units Petition
For financial aid recipients who have failed to meet the SAP time frame requirement. Students disqualified for exceeding time frame requirements may appeal by submitting an Excess Units Petition. Deadline for submitting petition: October 30 for Fall; March 30 for Spring.

For financial aid recipients who have failed to meet SAP criteria after a WARNING period due to serious illness or injury, death of an immediate family member, or other catastrophic or compelling situation beyond their reasonable control.
